We are grateful to all those who contributed to this work, especially those who volunteered as peer support facilitators. General practices across Cambridgeshire, Essex, and Hertfordshire supported this work, with assistance from the National Institute for Health Research. The specialist dietitians and diabetes nurse and Cambridge University Hospitals NHS Foundation Trust helped prepare resources for the project. Alfie Dale (theadditive.co.uk) prepared the video clips. NHS Cambridgeshire and Peterborough hosted the research grant on behalf of the University of Cambridge and Cambridge University Hospitals NHS Foundation Trust.

This toolkit presents independent research funded by the National Institute for Health Research (NIHR) under its Research for Patient Benefit (RfPB) Programme (Grant Reference Number PB-PG-0610-22311). The views expressed are those of the author(s) and not necessarily those of the NHS, the NIHR or the Department of Health.

The toolkit also draws on materials produced as part of the RAPSID trial of peer support in diabetes, funded by Peers for Progress.

The materials (unless copyright is indicated on particular resources) may be used with appropriate acknowledgement.
